If you have a doctor’s prescription for a DME product please feel free to fax in your Rx before you come, this way we can have eligibility checked for you before you arrive.

If you can’t make it we can prepare everything for your delivery*.

If you are faxing a Prescription to us for the first time please ensure that along with your prescription you include:

• Your First and Last Name
• Patient Information (Date of Birth, Home Address and a Number we can best contact you)
• Insurance information (for Medicare Beneficiaries please include your Part B card information)
• Doctor Prescription
